ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
- The Social Worker is responsible for assessing the psychosocial status of hospice patients and their family/caregiver(s) related to the patient’s terminal illness and environment. Communicates findings to the registered nurse and other members of the interdisciplinary team.
- Conducts safety assessments, social evaluations, caregiver abilities, communication patterns, and risk factors associated with neglect or abuse among others concerns, to inform intervention plans based on findings.
- Counsels’ patients and family/caregiver(s) in relationship to stress and other identified coping difficulties. Educates patients and families on, and assists in, preparation of advanced directives. Provides crisis intervention when called for.
- Educates interdisciplinary group of any special needs related to the culture of the patient and family, considerate of role of the family, space, and any special traditions or taboos.
- Contributes to patient care planning and collaborating with a variety of community resources and partners to connect our patients and their families with resources to help in identified areas of need.
- Contributes to the development of the individualized plan of care, involving the patient and family/caregiver(s), and may assist the physician and other team members in recognizing and understanding the effects of the psychosocial stresses on the symptoms of the terminal illness, as well as social and emotional factors related to health problems and death/dying issues.
- May contribute to, or asked to take part in, quality assessment performance improvement teams and activities.
- May assist family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities.

POSITION QUALIFICATIONS
- Licensed Social Worker designation (master’s degree in social work from an accredited school of Social Work)
- Minimum of one (1) year of experience in health care, preferably within a hospice or home health setting.
- Understanding of hospice philosophy and issues of death/dying.
- Demonstrates good verbal and written communication and organizational skills.
- CPR Certification required.
- Must be a licensed driver with a vehicle in good working condition and auto insurance per the state’s minimum coverage requirements.
Pay: $85.00 per hour
Expected hours: 1 – 40 per week
